Position: Licensed Social Worker Hospice
Shift: Day Shift
Scheduled Weekly Hours: 40
Department: Hospice

Position Overview:

Provides professional social work services through comprehensive assessments and effective resolution of issues affecting patients' overall health and well-being. This includes discharge planning and support during emotionally and medically complex situations, with a focus on hospice care.

Essential Duties:

  • Provide supportive counseling, crisis intervention, and facilitate therapeutic, support, and educational groups as needed.
  • Conduct psycho-social assessments using a systems-based approach.
  • Assist patients with Advance Directives and complete required screenings (e.g., Pre-admission Screening for Mental Illness and Intellectual Disability).
  • Practice independently in accordance with Ohio licensure; may supervise LSWs, LPCs, and students when delegated by a manager.
  • Deliver appropriate interventions to patients and families during times of psychosocial, emotional, or financial stress.
  • Act as an advocate and mediator for patients, supporting their right to self-determination with compassion and sensitivity.
  • Involve patients and families in decision-making regarding care plans.
  • Manage complex family dynamics and deliver high-level patient and family interventions.
  • Support orientation and training for new staff and students.
  • Monitor length of stay and contribute to strategies that minimize unnecessary resource utilization.
  • Collaborate with physicians and the interdisciplinary team to ensure high-quality, coordinated care.
  • Stay informed on laws and regulations affecting social work practice.
  • Work closely with medical staff, nursing, allied health, and RN Case Managers to support positive patient outcomes.
  • If cross-trained and credentialed, may function as a Behavioral Health Therapist or Chemical Dependency Counselor.
  • Adhere to the ethical standards established by the State of Ohio Counselor and Social Work Board.

Qualifications:

  • Master's degree in Social Work or Counseling with active licensure in Ohio. (Hospice/Home Care requires MSW.)
  • Strong verbal and written communication skills.
  • In-depth understanding of human behavior and personality development.
  • Ability to apply a holistic approach to patient care.
  • Valid driver's license and proof of auto insurance.
  • Familiarity with community resource networks.
  • Knowledge of Medicaid waiver programs and ability to make appropriate referrals.
  • Understanding of the signs of abuse/neglect and compliance with reporting laws and organizational policies.
