Job DescriptionSalary Range: $65,000 - $78,000 / yearThe LADC II position is contingent upon grant funds, employment will depend on the successful acquisition and continuation of the behavioral health grant.The LADC II provides comprehensive substance use disorder and behavioral health services within a SAMHSA grant–funded, integrated primary care and behavioral health program at a Community Health Center. This role delivers counseling, crisis intervention, and care management services to patients across the continuum of substance use, including those who are not abstinent, using a harm reduction and recovery-oriented framework. The counselor works collaboratively within a multidisciplinary healthcare team to address complex medical, behavioral health, and psychosocial needs.Licensed Alcohol and Drug Counselor II (LADC II)Essential Duties and Responsibilities:
- Clinical, Counseling & Crisis Services
- Conduct comprehensive substance use disorder intakes and assessments, including ASAM Biopsychosocial Assessments
- Provide individual counseling using a versatile, patient-centered approach, incorporating Motivational Interviewing, CBT, trauma-informed care, relapse prevention, and harm reduction strategies
- Deliver crisis intervention, safety planning, and risk assessments for patients experiencing acute emotional distress, relapse, or psychiatric instability
- Engage patients who may be actively using substances, meeting them where they are and promoting safety, engagement, and incremental change
- Treatment Planning, Care Management & Coordination
- Develop, implement, and update individualized treatment plans aligned with patient goals, readiness for change, and medical/behavioral health needs
- Provide care management services, including:
- Coordinating appointments across primary care, behavioral health, and specialty services
- Supporting linkage to higher or lower levels of care as clinically indicated
- Assisting patients with navigation of healthcare systems and community resources
- Collaborate closely with primary care providers, addiction medicine, psychiatry, recovery coaches, case managers, and external partners
- Participate in multidisciplinary team meetings, case reviews, and integrated care planning
- Compliance, Ethics & Documentation
- Maintain strict adherence to HIPAA and ROI requirements, ensuring ethical and appropriate information sharing
- Complete timely, accurate, and compliant documentation in the electronic medical record
- Ensure documentation meets SAMHSA grant, BSAS, payer, and Community Health Center standards
- Maintain professional boundaries and ethical clinical practice
- Program & Team Contribution
- Work effectively in a fast-paced, high-acuity integrated care setting, requiring strong multitasking and prioritization skills
- Demonstrate flexibility, sound clinical judgment, and accountability
- Contribute to program development, quality improvement initiatives, and team-based workflows
- Maintain clear, ongoing communication with team members to support coordinated, whole-person care

- Experience in a Community Health Center or integrated care setting
- Experience with electronic medical records, Excel data, state ESM data, and federal RedCap data
- Familiarity with SAMHSA grant-funded programs
- Experience working with co-occurring mental health and medical conditions
- Comfort working with patients at varying stages of change
- Bachelor’s degree in counseling, social work, psychology, human services, or related field (Master’s degree preferred)
- Experience providing substance use disorder counseling, crisis intervention, and care management
- Strong knowledge of harm reduction principles and integrated primary care models
- Ability to work collaboratively within a multidisciplinary healthcare team
- Excellent organizational, multitasking, and documentation skills
