Our organization is in search of a dedicated and skilled Registered Nurse to become a vital member of our Healthcare & Medical Services team at the Health Center in Vermilion, OH. In this role, you will be instrumental in providing exceptional, patient-focused care to individuals in our assisted living, rehabilitation, and memory care facilities.
Key Responsibilities
- Conduct thorough nursing assessments and create tailored care plans.
- Administer medications and treatments while closely monitoring residents' health to ensure their safety and comfort.
- Collaborate with physicians, therapists, nursing assistants, and other interdisciplinary team members to coordinate care.
- Communicate effectively and respectfully with residents and their families, providing education on diagnoses, medications, and ongoing care requirements.
- Maintain accurate and timely documentation in the Electronic Medical Records system.
- Adhere to infection control and safety protocols, actively participating in quality improvement initiatives.
- Support residents through rehabilitation and long-term care, particularly those with complex medical conditions and memory-related issues.
- Respond promptly to changes in residents' conditions, performing clinical triage and prioritizing care as needed.
- Mentor and assist nursing assistants and other team members, fostering a positive and educational environment.
Requirements and Qualifications
- Current Registered Nurse license.
- Compassionate approach to patient care.
- Strong clinical assessment and critical thinking skills.
- Ability to work collaboratively within a team.
- Excellent communication skills, both verbal and written.
Working Conditions
This is a full-time, on-site nursing position that may require rotating shifts, including weekends and holidays, to ensure continuous care for our residents.
