Community Outreach Assistant
PRN Part-Time Independent Contractor | Hybrid | Maryland-Based Hire Only
Compensation: $17.00–$17.50 per hour, based on experience and qualifications.
About the Role
We are a mission-driven, Maryland-based non-medical home care agency and senior care placement assistance provider dedicated to helping individuals remain safe, independent, and comfortable in their homes.
Our work supports smooth transitions from facility to home and helps connect clients with high-quality, reliable care in the community. We are seeking a professional and personable Community Outreach Assistant to help our home care agency build relationships with healthcare facilities and community partners.
This role focuses on outbound outreach and partnership development with healthcare partners.
Responsibilities
Responsibilities include, but are not limited to:
- Serve as a liaison between the agency and healthcare community partners to support smooth transitions from facility to home.
- Conduct inbound and outbound calls and send follow-up emails to healthcare facilities and community organizations.
- Introduce the agency as a potential home care resource for patients transitioning to home.
- Build and maintain professional relationships with discharge planners, social workers, case managers, referral coordinators, and healthcare partners.
- Communicate by phone and email to inquire about referral processes and preferred provider or community resource lists.
- Identify partnership development and referral opportunities through phone and email outreach.
- Maintain organized outreach documentation and complete weekly activity tracking.
- Submit weekly outreach summaries and updates by email.
- Attend required team meetings and participate in in-person meetings or training as applicable.
Weekly Expectations
- Maintain consistent outbound outreach, follow-ups, and email communication with healthcare partners.
- Professionally represent the agency in all phone and email communication.
- Maintain accurate documentation, email tracking, and follow-up logs for contacts and activities.
- Support long-term partnership development and referral relationship growth.
Qualifications
- Must be at least 18 years of age
- Local Maryland hires only; candidates must reside in Maryland and be available for in-person meetings and training.
- 1–2 years of experience in healthcare, home care, healthcare marketing, community outreach, customer service, or a related field preferred.
- Call center, outbound calling, sales, business development, or appointment-setting experience is a plus.
- Strong communication and relationship-building skills.
- Professional phone etiquette and confidence communicating with healthcare professionals and administrative staff.
- Organized, detail-oriented, and able to maintain accurate outreach records.
- Comfortable making outbound calls and conducting professional follow-up by phone and email.
- Reliable internet and computer access.
- Proficiency in Microsoft Excel, Word, email, Office 365, and other technology platforms used for communication and documentation.
- Ability to maintain confidentiality.
- Must successfully complete all required agency onboarding, training, and compliance requirements.
Position Details
- PRN part-time independent contractor position.
- Hybrid position, in-person meetings or training as applicable.
- Up to 6 hours per week; hours are not guaranteed.
- Flexible scheduling within the agency's designated weekday daytime outreach hours.
- Consistent weekday daytime availability.
- Calls are made through the company's phone system.
- Maryland-based applicants only.
Additional Terms
- Position includes an introductory contract period of up to 10 days.
- Must successfully pass a background check prior to starting.
- Must complete required medical screening and documentation, which may include a physical examination and tuberculosis (TB) screening, as applicable to the position and agency requirements.
- Drug screening may be required, subject to agency policy and applicable requirements.
- Contractor must sign a confidentiality agreement.
- Must successfully complete all required agency onboarding, training, and compliance requirements before performing assigned outreach activities.
Benefits
- No employee benefits are provided during the independent contractor period.
- Benefits, if applicable, may be offered if the contractor is later hired into an eligible part-time employee position.
