Description:
The City of Fishers is seeking a full-time Public Health Nurse (PHN) – School Liaison to join our Health Department. This role is supported by Health First Indiana funding. As the designated school liaison, this role will work closely with Fishers K-12 schools, preschools and daycares, to support Fishers schools in public health response and prevention activities. This role is also a member of the public health nursing team, which includes providing clinical and case management support for other FHD programs.
Examples of Duties:
- Serve as the primary public health resource for all public, private, and charter K-12 schools, plus preschools and daycares
 - Collaborate on student health and wellness initiatives, identifying opportunities to enhance school health programs
 - Ensure schools meet health-related requirements including vision/hearing screenings, dental health and immunizations.
 - Conduct quarterly school meetings and attend state-level school liaison sessions
 - Assess school health needs and develop comprehensive plans to address identified
 - Educate families on child development, immunizations, and preventive health
 - Create individualized health care plans supporting client's unique needs
 - Conduct home visits to monitor individual and family health needs
 - Deliver educational resources on infectious disease prevention and health promotion
 - Connect families with appropriate community resources and services
 - Participate in public health emergency response activities
 - Support other public health programs and initiatives as assigned
 - Adapt to changing public health priorities and emerging community needs
 - Contribute to special projects and cross-functional team efforts
 - Embrace new responsibilities that enhance community health outcomes
 - Maintain accurate patient records and documentation
 - Ensure HIPAA compliance and confidentiality
 - Manage medical supply inventory
 - Stay current with best practices through professional development
 
Minimum Qualifications:
- Highly prefer bachelor's degree in nursing but would consider an associate degree;
 - Must possess an active RN licensure by the State of Indiana;
 - Must have an active IN Driver's License with a record that allows the ability to be insured to drive a city vehicle;
 - Must have access to personal vehicle to use throughout the work day and current automobile insurance;
 - Must have at least 1 year of experience working directly with patients and medical providers as a nurse with a preference for those with experience in school-based nursing and/or pediatric nursing;
 - Role requires the ability to communicate clearly and concisely, both orally and in writing to a variety of audiences including those who are non-native English speakers;
 - This position is required to maintain all required licenses and certifications;
 - Ability to successfully complete both the National Incident Management System (NIMS)/Incident Command System (ICS) 100, 200, 700 and 800 certification and CPR certification within four (4) months of employment;
 - Occasional evenings and weekends are required. Typically, Nurses are expected to work one evening shift per week (to 7:30pm) and one weekend shift per month (Saturday 8am - 1pm). This schedule could vary based on business needs.
