Pay: From $48.00 per hour
Expected Hours: 15–30 hours per week
Work Location: Hybrid / Remote (with in-home client visits)

Eldercare Navigators serve as trusted liaisons between clients, families, and healthcare providers with the goal of improving quality of life and ensuring continuity of care as individuals age.

Navigators fill gaps in the healthcare system to ensure that every client’s Power, Control, Dignity, and Peace of Mind are preserved and respected. The position requires strong communication skills, emotional intelligence, and a thoughtful, client-centered approach to complex family dynamics and healthcare transitions.

We are seeking Licensed Social Workers who:
- Have a passion for working with older adults
- Understand the long-term care continuum
- Can confidently navigate emotionally sensitive conversations
- Are proactive, resilient, and adaptable
- Balance immediate needs with long-term planning
- Are trustworthy, patient, and open to continuous learning
Key Responsibilities
- Visit clients in their homes and accompany them to medical appointments
- Conduct comprehensive assessments and develop mature life plans
- Provide care coordination, crisis management, and advocacy
- Assist with medication management and advance directives
- Support transitions to and from care facilities
- Communicate clearly with clients, families, and healthcare providers
- Monitor care plans and anticipate future needs
- Educate clients and families on care options and resources
Training & Support
- Initial training conducted at our main office in Somerville, NJ
- Shadow experienced Navigators in client homes to learn NightingaleNJ’s culture and values
- Flexible scheduling once working independently
- Assigned primary clients with full team support and backup coverage
- Weekly 1:1 check-ins and monthly team meetings
- Ongoing education and professional development opportunities

Requirements
- Active Licensed Social Worker (LSW) – required
- Experience working with older adults or in the long-term care continuum preferred
- Strong interpersonal, organizational, and communication skills
- Ability to work independently and collaboratively
- Comfortable with in-home visits and hybrid remote work
Compensation & Benefits
