Highland Hills Post Acute is a 200-bed post-acute care facility located at 1105 Perry Highway, Pittsburgh. Our dedicated staff members are ready to welcome you to the team. Successful candidates will have the following:
- Social services experience in a skilled nursing facility
- Degree in Human Services or associated field is a plus, but not mandatory
- Experience with PCC is ideal
- Knowledge of assistance programs is helpful
Your day to day:
You'll provide assistance in caring for our residents by working with our Social Services Director and physicians to create care plans that best suit the resident's needs. You'll monitor and document the emotional, physical, and mental wellbeing of the residents and ensure their needs are met. You'll work under supervision of our Social Services Director to ensure compliance with current state, federal, and local standards.
