Job Summary

Qualifications
- Active & Unencumbered LPN License (In Good Standing with the Board of Nursing)
- Minimum 1 Year of Experience in Long-Term Care (LTC), Nursing Homes
- Graduation from an Accredited LPN Program
Key Responsibilities
Provide compassionate bedside care in skilled nursing settings
Administer medications and treatments per physician orders
Monitor patients’ health status and report changes to RNs and supervisors
Collaborate with the healthcare team to create personalized care plans
Ensure accurate and timely documentation according to state and federal regulations
Comply with State Board of Nursing continuing education requirements
