Schedule & Compensation
Hours: Monday–Friday, daytime shifts, every-other weekend work requirement
Pay: Based on experience

What You'll Do
As the LPN Care Coordinator, you'll play an important part in ensuring residents receive exceptional care each day. Your responsibilities include:
This position will also require shifts working in direct care as well.

Providing hands-on nursing care including medication administration, treatments, and ADL support

Working closely with the RN and team leads to support individualized care plans

Completing documentation with accuracy and attention to detail

Supporting Resident Assistants through mentorship and coaching

Managing medication orders, refills, and overall medication processes

Performing supervisory visits, completing chart reviews, and ensuring compliance with company policies and our Medicare Comprehensive License
What We're Looking For

Current Minnesota LPN license
-
Strong communication skills with the ability to work professionally with staff, residents, and families

Experience in Assisted Living highly preferred

Familiarity with Alzheimer's/dementia care is a plus

Confidence using electronic documentation systems
