We are looking for a teammate in Brentwood, Tennessee who has a heart for the senior population and who is dedicated to providing the best care possible to our aging population. This role will require you to be the face of the operation within the community in which you operate. You’ll consult with the community operator, the clinician, the seniors, and even their families to help them navigate their unique healthcare experience. You understand that these experiences might vary from person to person, and you take stock of their unique, individual goals when making recommendations. Some days you might be happily educating or assisting seniors or their families in understanding how to use an application or a tablet. Others, you might be organizing health education for the communities to learn more about the common conditions that affect them. Our Navigator is an agile, warm and caring, self-motivated person who does not shy away from a challenge; Instead, they seek solutions with a passion for the connected well-being of the population that we all serve.
Join our DYNAMIC team as a Health & Wellness Coordinator! The HWC is a nurse leader who supports clinical operations alongside our Health & Wellness Director.
Our ideal teammate would be able to:
- Works with caregivers to ensure the principles of assisted living and Bickford’s culture are integrated into all aspects of resident care.
- Maintain regulatory compliance.
- Ensure residents receive medications/treatments as prescribed by their physician.
- Coordinate health care services with external care providers and physicians to ensure resident needs are met.
- Communicate with seniors and their families to ensure their health and wellness plans are tailored to their individual goals
- Facilitate the patient’s transition within and between health care settings - Transitions of care between Assisted living, inpatient hospital (when necessary), skilled nursing rehabilitation (when necessary), and back home to their assisted living apartment
- Facilitate and collaborate with staff, patient, and family decision making for their healthcare journey
- Collaborate with multidisciplinary team to coordinate appropriate plan of care
- Facilitate educational seminars for patients about self-management of acute or chronic illness
- Schedule and plan health and wellness programming, including integrated clinical service partners (i.e., vaccine clinics, Exer.ai gait events, pharmacy Part D education events, etc.)
- Assist seniors and their families with technology when requested or when a need is observed
- Champion technology and solution integrations within the community
- User and resident management of systems within the community
- Ensure data completion and quality in systems (EHR, Sage, etc.)
- Drive quality metrics and results
- Perform other duties as assigned
Would you describe yourself as someone who has:
- Active Tennessee LPN (preferred) or RN License
- Willing to take additional training or education, which specific states may require
- CPR and First Aid certifications are required
- Ability to handle multiple priorities
- Background in Program Management
- Strong Computer skills with understanding of EMR
- Experience working within a senior community
- Good understanding of Medicare and insurance within the USA
- Experience working in the customer service industry a plus
- A warm and friendly countenance with a solution-oriented mindset
- Knowledge of chronic diseases and care management of those diseases
- Passion for seniors with complex chronic and mental health conditions; especially those with dementia
