Key Responsibilities

Provide compassionate nursing care and monitor residents’ overall health and well-being.

Assess residents’ conditions, check vital signs, and report changes promptly to the appropriate nursing supervisor.

Administer medications and treatments according to physician orders and facility policies.

Initiate, review, and update individualized care plans.

Supervise and support Certified Nursing Assistants (CNAs), ensuring nursing policies and procedures are followed.

Assist with resident admissions, discharges, and transfers as needed.

Maintain accurate and timely clinical documentation.

Communicate effectively with physicians, residents, families, and interdisciplinary team members.

Maintain confidentiality of resident health information in accordance with HIPAA requirements.

Follow infection control procedures and applicable healthcare regulations.

Perform additional duties as assigned by nursing or facility leadership.
Requirements
-
Current, valid Licensed Practical Nurse (LPN) license in South Carolina.

Strong clinical assessment and communication skills.

Ability to work collaboratively with nursing staff and other healthcare professionals.

Compassionate, resident-focused approach to care.

Strong organizational skills and attention to detail.
