In this role, you’ll perform the core responsibilities of an LPN in a long‑term care environment with an emphasis on continuity, safety, and individualized care. You will administer medications, monitor vital signs, assist with activities of daily living, manage wound care, and participate in or lead care planning under established protocols. You’ll collaborate with registered nurses, physicians, therapists, and certified nursing assistants to ensure timely interventions, accurate documentation, and rigorous adherence to regulatory standards. Your duties will include accurate MAR charting, administering approved treatments, recognizing early signs of decline, and communicating changes to the care team and families with empathy and clarity.
