Job DescriptionJob Title: Medical Case Manager – Trauma Patients
Location: Emeryville, CA
Employment Type: Direct Hire
Salary: $80,000 – $95,000 annually (DOE)

Overview:
We are seeking a compassionate and experienced Medical Case Manager to join a clients dedicated healthcare team in Emeryville, CA. This role focuses on managing and monitoring trauma patients throughout their recovery journey, ensuring they receive coordinated, high-quality care.

Key Responsibilities:

Manage a caseload of trauma patients, coordinating care plans and follow-up services
Monitor patient progress and advocate for necessary medical and support services
Collaborate with physicians, nurses, social workers, and insurance providers
Maintain accurate documentation and ensure compliance with healthcare regulations
Provide emotional support and guidance to patients and families
Qualifications:

Bachelor’s degree in Phycology, Nursing, Social Work, Health Administration, or a related field (required)
Minimum 3 years of recent, stable experience in case management or trauma care
Strong understanding of medical terminology, care coordination, and patient advocacy
Excellent communication, organizational, and problem-solving skills
Passion for helping patients navigate complex medical systems
