Job Overview:

Our comprehensive approach blends medication managementincluding evidence-based options like buprenorphinewith interventional therapies and mental health support to foster recovery and wellness. We currently have openings at our Minneapolis location and invite motivated professionals to help us continue making a positive difference in our communities.
Primary Responsibility:
Evaluation
- Obtain medical history from patients
- Perform evaluation, delineate problems, record and present data
Monitoring
- Assist in provision of continuity of care
- Record progress notes
Diagnostics
- Interpret and evaluate patient data
- Order and/or perform diagnostic procedures
Therapeutics
- Perform routine procedures
- Develop and implement patient management plans
- Counseling
Instruct and counsel patients regarding the following:
- Emotional problems
- Prescribed therapeutic regimes
Referral
- Facilitate the referral of patient to other health care providers and specialists when appropriate
Counseling
- Instruct and counsel patients regarding emotional problems and prescribed therapeutic regimes
Preferred Skills and Knowledge:
- Demonstrated clinical expertise with patients with complex medical issues of all age groups
- Skills normally expected to diagnose and treat.
- Excellent communication skills
- Ability to coordinate patient care in multiple settings
- Ability to work independently
- Flexibility in job responsibilities
- Strong collaborative background
Required Education, Experience and Credentials:
Masters degree as an adult or family NP, or in Physician Assistant Studies.
Current MN license as either a nurse practitioner or physician assistant.
Current board certification
Current DEA certification
Current ACLS
Additional Position Details: (hours, locations, commute)
- Full-time, Monday-Thursday, can add Fridays based on FTE
- Day Shift; No on-call or weekends
