Licensed Master Social Worker (LMSW)
Location: New York, NY
Type: Contract | 13 Weeks
Pay:
Local Rate: $2,200 – $2,400 Weekly
Travel Rate: $2,700 – $2,950 Weekly
Shift & Schedule: Monday – Friday | 9:00 AM – 5:00 PM | 40 Hours Weekly
Position Overview
We are seeking an experienced Licensed Master Social Worker (LMSW) to provide inpatient social work services within a fast-paced acute care hospital environment. The LMSW will support patients and families through psychosocial assessments, discharge planning, care coordination, counseling, and community resource referrals.
The ideal candidate will possess strong hospital-based social work experience and the ability to collaborate effectively with interdisciplinary healthcare teams.
Responsibilities
• Conduct comprehensive psychosocial assessments for patients and families.
• Provide supportive counseling and therapeutic interventions.
• Coordinate discharge planning and continuity of care services.
• Facilitate referrals to community resources and support programs.
• Collaborate with physicians, nurses, case managers, and interdisciplinary teams.
• Assist patients and families with crisis intervention and care coordination.
• Maintain accurate clinical documentation and case records.
• Support patients through medical, emotional, social, and financial challenges.
• Participate in patient care conferences and interdisciplinary rounds.
• Ensure compliance with hospital policies and regulatory standards.
Required Qualifications
Education
• Master's Degree in Social Work (MSW).
Licensure / Certification
• Active New York State LMSW License.
• Primary Source Verification required.
Experience & Requirements
• Minimum 1 year of LMSW experience.
• Strong discharge planning and referral coordination experience.
• Excellent communication, counseling, and documentation skills.
Preferred Qualifications
• Hospital Social Work experience.
• Experience in Med/Surg, Acute Care, Rehabilitation, or Inpatient settings.
• Familiarity with community resources and care transition programs.
