Livingston Memorial is looking for a Full Time, 40 hours a week, Clinical Manager to support our Palliative Care Team.

Position Summary:

The Clinical Manager’s key responsibilities are to plan, coordinate, manage and evaluate the activities of multidisciplinary teams to ensure the delivery of high-quality home care services to Palliative patients.

This includes supervision of assigned Case Managers, Registered Nurses, Licensed Vocational Nurses, Physical Therapist, Occupational Therapist, Physical Therapy Assistant, Social Worker and Certified Home Health Aides for Palliative patients. This position interacts with patients, caregivers, and their families, physicians, community organizations, and works collectively and collaboratively with LMVNA administrative personnel, Quality Improvement, Education, and Compliance Department to ensure safe, quality care for the patients. The Clinical Manager ensures high quality patient care in compliance with Title 22, Medicare, state licensure regulations, and the organization's policies and procedures.


What You'll Do:

  • Supervises and oversees the planning, organizing, and directing of Palliative patient care services, ensuring consistent delivery of high-value quality care.
  • Establish the patient assignments, staff scheduling, and patient visit volume distributions, and assists in meeting financial targets by effectively managing utilization, productivity, personnel, and supplies.
  • Supports and educates staff in resolving operational issues/problems.
  • Assists in developing and implementing changes in policies, practices, and standardized procedures based upon input from staff and administration to remain congruent with current laws and regulatory standards while promoting optimal patient care.
  • Collaborates with others to coordinate services, streamline and standardize work flows and procedures, and/or drive positive outcomes.
  • Performs field supervision, evaluations and audits to ensure physician orders are followed by staff and visit documentation is done appropriately.
  • Responsible for review of the appropriate number of Palliative Case Managers and clinical staff documentation to include starts-of-care, resumption-of-cares, and re-certifications, for appropriateness of care, delivery, and documentation requirements.
  • Assesses staff education needs based on the review of clinical documentation in addition to feedback and recommendations by Utilization Review staff. Upon completion of the assessment, creates and conducts regular staff education as needed.
  • Oversees and coordinates the transition of patients from palliative care to hospice, ensuring timely evaluation, communication, and documentation to support safe continuity of care. Ensures the transition process is efficient, patient‐centered, and compliant with regulatory and eligibility requirements.
  • Provides direct patient care on an infrequent basis as needed.
  • Manage referrals, assign patients to teams, oversee patient care plans.
  • Train, and evaluate clinical staff to ensure effective performance.
  • Participates in the Administrator On Call rotations.
  • Participates in the interviewing, hiring, training, and development of direct care clinicians.
  • Participates in case conferences and staff meeting. Supervises and coordinates the activities of the patient care team to ensure collaboration and coordination of care for the patients.
  • Participates in orientation of new staff and assignment of clinical preceptors.
  • Serves as clinical resource for team members including, but not limited to, responding to clinical questions, obtaining needed orders and/or facilitating interdisciplinary communication to ensure collaborative problem solving.
  • Demonstrates leadership skills that are aligned with the strategic plan in coordination with LMVNA leadership.
  • Demonstrates ability to handle sensitive issues and conflict among others and recognizes when to consult with the Director of Patient Care Services.
  • Responsible for the QA/PI activities. Works with Quality team relative to data tracking for performance review and outcomes of care analysis to determine efficiency, the efficacy of case management system as well as any other systems and process. Competently performs patient care assignments and staff management activities.
  • Assures, in collaboration with Case Manager, the accurate and compliant completion of all clinical documentation, including OASIS assessments, Plans of Care with clinically appropriate frequency and durations to meet the needs of the patient under the Conditions of Participation (CoPs).
  • Provides guidance and support to clinicians to ensure OASIS discipline‐specific accuracy, consistency, and timely completion.
  • Identifies documentation gaps or inconsistencies and works with clinicians to correct and strengthen clinical documentation in real time.

  • Actively participates in agency committees and project teams as assigned.
  • Performs other related duties as assigned.


What We're Looking For:

  • Valid California Registered Nurse License
  • Minimum four (4) years professional nursing experience
  • Minimum three (3) years Home Health experience.
  • Valid Basic Life Support Certification
  • Valid Driver License and auto insurance
  • Knowledge of computer, proficient with Microsoft Word and Excel.
  • Good verbal and written communication skills.
  • High level of attention to details.
  • Able to work independently, take ownership and responsibility of tasks.
  • Strong professional ethics, discretion and judgment.
  • Good organizational skills.
