Medical Social Worker (MSW) – Contract to hire

Location: Detroit, MI 48202
Pay Rate: $45–$50/hour
Contract Length: 13 weeks (Contract to hire)


Job Summary

We are seeking an experienced Medical Social Worker (MSW) to provide direct social work services to pediatric and adolescent patients in a home care and hospital setting. This role involves supporting students and families with personal, environmental, and social challenges while collaborating closely with healthcare providers and school staff to deliver comprehensive care.


Key Responsibilities

  • Provide direct social work services to pediatric/adolescent populations
  • Implement all five phases of the social work process
  • Collaborate with students, families, and school staff to develop effective care plans
  • Coordinate care with interdisciplinary teams to ensure quality outcomes
  • Assist with discharge planning and care transitions
  • Provide referrals to community resources and external agencies
  • Communicate effectively to support patient satisfaction and goal achievement


Required Qualifications

  • Master’s Degree in Social Work (MSW) from an accredited program
  • Current Michigan LMSW or LLMSW license
  • Minimum 5+ years of case management experience
  • At least 2+ years of recent acute inpatient case management experience (REQUIRED)
  • Strong experience in discharge planning
  • Valid Michigan driver’s license and reliable, insured vehicle
  • Ability to work independently with minimal training and adapt quickly

⚠️ Important:
Candidates without recent inpatient case management experience will not be considered. Experience in telephonic case management, utilization review, or purely psychological roles does NOT meet this requirement.


Preferred Qualifications

  • Previous home care experience
  • Experience working with pediatric and adolescent populations


Schedule

  • Shift Options:
    • 8-hour shifts:
      • 8:00 AM – 4:30 PM
      • 3:00 PM – 11:30 PM (Emergency Department support)
    • 10-hour shifts:
      • 3:00 PM – 1:30 AM (Emergency Department support)
  • Weekend rotation required
  • No holidays


Additional Notes

  • Limited on-site training provided – must be able to hit the ground running
  • Manager will contact candidates directly for interviews
  • Candidates in “Client Review” status should be prepared for outreach
