Key Responsibilities
Provide skilled nursing care to ill, injured, or disabled patients in their homes.
Conduct comprehensive patient assessments and create individualized care plans.
Monitor and report any changes in a patient's symptoms or overall condition.
Communicate with collaborating physicians and specialists to coordinate patient care.
Educate patients and their families on health maintenance, disease prevention, and proper care techniques.
Facilitate referrals to other healthcare professionals and medical facilities as needed.
Maintain accurate and detailed patient medical records.
Offer emotional support and guidance to patients and their family members.
Qualifications
Current and valid Registered Nurse (RN) license.
Proven experience in nursing, preferably in a home health, hospice, or other relevant medical setting.
Familiarity with medical software and equipment.
Excellent interpersonal skills and the ability to build rapport with diverse patients and families.
Strong problem-solving and critical-thinking abilities.
Ability to work independently and manage time effectively in a fast-paced environment.
