Position Overview: We are seeking a dynamic and compassionate Care Manager Business Development to join our team. This hybrid role combines expertise in client care coordination with strategic business development efforts to grow our client base. The ideal candidate will have a background in home care, healthcare, or senior services and a passion for helping others, along with strong networking and relationship-building skills.
Key Responsibilities:
Client Intake & Care Coordination (50%)
- Serve as the primary contact for new client inquiries; conduct in-home assessments to evaluate care needs.
- Develop personalized care plans and match clients with appropriate caregivers.
- Provide ongoing support to clients and families, ensuring satisfaction with services.
- Collaborate with the scheduling and care teams to ensure seamless service delivery.
Business Development & Outreach (50%)
- Identify and cultivate relationships with referral sources including hospitals, rehab centers, senior living communities, case managers, social workers, and physicians.
- Attend community events, health fairs, and professional networking meetings to promote agency services.
- Track outreach efforts and maintain a pipeline of potential client leads.
- Develop and implement strategies to increase brand awareness and grow the client base.
Qualifications:
- Bachelors degree in Healthcare, Social Work, Business, or related field (or equivalent experience).
- Minimum 2 years of experience in home care, senior services, or healthcare sales/marketing.
- Excellent interpersonal, communication, and presentation skills.
- Self-starter with a results-driven mindset.
- Reliable transportation and willingness to travel locally.
