Physical Therapist
- Promotes Elara Carings philosophy, mission statement and administrative policies to ensure quality of care.
- Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
- Reviews patients functional needs and adjusts care plans to ensure optimal patient outcomes.
- Designs and implements a plan of care for patients based on assessment and includes participation by significant others as appropriate.
- Conducts therapy practice within defined standards of care.
- Directs assistance of support staff to achieve patients functional goals.
- Documents evaluations, treatment goals, and plans; regularly updates documentation and maintains prescriptions and signatures.
- Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community.
- Physically demanding environment requiring full range of body motion including handling, lifting and transferring patients.
- Potentially irregular hours including call hours if applicable.
Requirements:
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted PT license valid for the state of work.
- Minimum one year of experience in a healthcare facility.
- Able and willing to travel within branch/office coverage area (up to 50%).
- Dependable vehicle, valid drivers license, and current auto insurance per state laws.
- Ability to frequently lift, push, pull, and support up to 50 pounds.
