JOB SUMMARY:
We are seeking a compassionate and organized Patient Care Coordinator to join our healthcare team. The ideal candidate will be responsible for ensuring smooth and efficient patient care coordination, from scheduling appointments to managing patient communications. This role is pivotal in helping patients navigate the healthcare system, ensuring they receive the care they need in a timely and compassionate manner.

ESSENTIAL JOB FUNCTIONS:
- Patient Appointment Coordination: Schedule and confirm patient appointments, ensuring that appointments are scheduled appropriately, and all necessary pre-appointment paperwork is completed.
- Patient Communication: Serve as a primary point of contact for patients, answering inquiries, addressing concerns, and providing assistance with appointment scheduling, test results, and follow-up care.
- Medical Records Management: Maintain accurate and up-to-date patient records, ensuring all information is entered into the electronic health records (EHR) system in a timely and confidential manner.
- Insurance Verification: Verify insurance information and eligibility, assist with insurance authorizations, and address insurance-related concerns or inquiries.
- Patient Advocacy: Assist patients with navigating their healthcare plans and ensure they understand their care plans and treatment options.
- Collaboration with Medical Staff: Work closely with physicians, nurses, and other healthcare professionals to ensure that patient care is seamless and that all necessary procedures, tests, and referrals are handled.
- Follow-Up Care: Ensure that patients are reminded of upcoming appointments, necessary follow-up visits, or tests. Help coordinate post-visit care, referrals, and other follow-up procedures.
- Data Entry and Reporting: Input and maintain accurate patient data in the practice's EHR system, and generate reports as required by the healthcare team.
Other Duties:
- Assist in managing day to day operations of the clinic and providers general administrative duties which may include answering and directing phone calls, scheduling appointments, responding to the general administrative inbox, checking phone messages and faxes, managing clinic schedules, and ordering supplies as needed.
- Serves visitor by greeting them, welcoming and directing them appropriately. Assists clinic staff by managing the flow of patients.
- Maintains safe and clean reception area.
- Deals with queries from the public and patients appropriately.
- Call patients to make appointment reminders daily.
- Assists in managing the clinic staffs' schedules (blocking time out of the clinic in EMR, etc.)
- Manages the tracking of missed appointments and appointment not able to accommodates, etc.
- Trouble shoots daily obstacles within the clinic.
- Collects access fee for services, when applicable.
- Assists in maintaining medical records and checking patient eligibility.
- Assists in planning specialty events such as flu shots clinics and health screening events.
- Supports client wellness programing and acts as a liaison to the Account Executive.
- Assists in the coordination of program promotion including distribution of promotional materials, such as; handouts, payroll stuffers, letters, flyers, etc. when appropriate.
- Follow all standard operating procedures in regards to Protected Health Information (PHI) in compliance with HIPAA regulations.

Requirements:
SKILLS & ABILITIES:
- Strong organizational and multitasking skills.
- Excellent communication skills, both written and verbal.
- Proficiency in using Electronic Health Records (EHR) systems.
- Knowledge of medical terminology and healthcare processes.
- Ability to work well under pressure and in a fast-paced environment.
- Strong attention to detail and patient-centered attitude.
- Ability to work effectively as a team member and cooperate in achieving company goals.
- Problem solving ability to seek solutions using appropriate methodologies.
- Proficient computer skills, including but not limited to: Microsoft Word, Ecel, Internet Explorer, and Outlook.
EDUCATION & CERTIFICATIONS :
- High school diploma or equivalent required. Associate’s degree or higher in healthcare-related field preferred.
- Certification in Medical Office Administration (e.g., CMAA) or Patient Care Coordination is a plus.
EXPERIENCE:
- Minimum of 1-2 years of experience in a healthcare setting, preferably in patient coordination, medical office administration, or a similar role.
- Bilingual (English/Spanish): Preferred but not required.
