Key Responsibilities

- Collaborate effectively with physicians, nurses, and interdisciplinary healthcare team members to ensure coordinated care
- Maintain detailed, accurate, and compliant clinical documentation in accordance with regulatory standards
- Travel to patients' homes throughout the service area to provide skilled physical therapy services
Required Qualifications
- Master's Degree in Physical Therapy from an accredited program (Doctor of Physical Therapy preferred)
- Minimum of 1 year of professional experience as a Physical Therapist in home health care setting
- Current, valid Physical Therapist state license in good standing
- Current CPR/BLS certification from an approved provider
- Valid driver's license with reliable transportation and ability to travel to patient homes throughout the service area
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities to build rapport with patients and families
- Proficiency with electronic medical records and documentation systems
