Care Coordinator
Schedule Coordinator – In-Home Care Services
Position Overview
We are seeking a compassionate, organized, and detail-oriented Patient Care Coordinator to join our team and support the delivery of high-quality in-home care services. This role is responsible for coordinating caregiver schedules, ensuring patients receive consistent care, and serving as a key connection between patients, caregivers, families, and the care team.
The ideal candidate will have excellent communication skills, strong attention to detail, and the ability to manage multiple priorities in a fast-paced healthcare environment.
Responsibilities:
- Coordinate and maintain caregiver schedules for in-home patients
- Match caregivers with patients based on care needs, availability, location, and skill set
- Manage daily scheduling changes, call-offs, coverage needs, and shift replacements
- Communicate with caregivers regarding schedules, assignments, and updates
- Communicate professionally with patients and families regarding service schedules and care coordination
- Monitor caregiver attendance and ensure scheduled visits are completed
- Maintain accurate patient and caregiver records
- Assist with onboarding and orientation coordination for new caregivers
- Document schedule changes and follow established procedures
- Work closely with the care team to ensure continuity and quality of services
- Provide excellent customer service to patients, families, and caregivers
Qualifications
- Previous experience in home care, healthcare scheduling, staffing, or a related field preferred
- Experience coordinating caregivers, aides, nurses, or healthcare staff strongly preferred
- Excellent organizational and time-management skills
- Strong problem-solving abilities and attention to detail
- Ability to remain calm and professional in urgent scheduling situations
- Strong verbal and written communication skills
- Proficiency with computers, email, scheduling systems, and Microsoft Office
Preferred Skills:
- Knowledge of home care operations and caregiver responsibilities
- Experience with scheduling software or electronic documentation systems
- Ability to build positive relationships with caregivers and patients
- Strong customer service mindset
