La Rabida Children’s Hospital provides specialized, family-centered health care to children with medically complex conditions, disabilities, and chronic illness. Through expertise, compassion, and advocacy we help children and their families reach their fullest potential, regardless of their ability to pay.

Our not-for-profit hospital, licensed for 49 beds, helps transition children from neonatal or pediatric intensive care to home, by providing medical, rehabilitative and developmental care, and by training families to continue treatments and manage the necessary equipment in the home. La Rabida also provides extensive rehabilitation for those recovering from wounds or burns and treatment for exacerbations of chronic conditions.

The hospital’s enhanced pediatric patient-centered medical home provides primary care to children with complex medical conditions and their siblings. Children with medical homes elsewhere come to La Rabida for specialty services. La Rabida offers a wide range of specialty services provided to children with sickle cell disease, diabetes, and many others. Children are supported in their emotional and developmental growth, particularly in cases where such growth has been interrupted by accident or disease.

Finally, La Rabida provides forensic and treatment services for children exposed to abuse and neglect, comprehensive assessments for youth in care, early intervention for children between 0 and 3 years of age. Care coordination services for medically complex children are also provided for those who are covered by a health plan and receive care from providers in Cook County

Job Description

The Registered Nurse (RN) will be responsible for working with and providing clinical support for technology dependent complex patients and their families in an outpatient setting. The RN will focus on Primary Care and Pulmonary needs in collaboration with a Primary Care Provider, Pulmonologist, Nurse Practitioner, Dietitian, Speech Therapist, Physical Therapist, Respiratory Therapist, and Social Worker.

  • Serves as clinical team liaison for families, care coordination entities, external agencies and other internal/external individuals providing services to assigned patient families.
  • Communicates essential information to patients, families, and members of the care team.
  • Acts as patient and family advocate
  • Provides education to patient/family as it relates to the provider prescribed treatment plan and nursing assessment of patient/family educational needs.
  • Conducts telephone triage and provides advice within nursing scope of practice and/or in collaboration with provider.
  • Provides intermittent/as needed patient follow-up between clinic visits.
  • Reviews and/or completes necessary paperwork related to patient/family needs. Paperwork includes but is not limited to home nursing care plans and orders, durable medical equipment paperwork, and school forms/orders.
  • Completes prior authorizations for medications as needed.
  • Responsible for NCQA care plan completion and management.
  • Delegates appropriate patient follow-up needs to the Access Center as requests are received.
  • Schedule patient appointments as needed.
  • Evaluates patients via intake process, in partnership with a care team to determine appropriateness of patients for services provided.
  • Facilitates clinic huddles with provider and clinic team, with advanced review of scheduled patients. Huddle includes clinical support services that may be needed, follow-up on labs, patient compliance with outside referrals, needed clinical care and quality gaps, and identification of patients ready to transition to adult care.
  • Documents all interventions completed as well as all team, provider and patient/family communication and concerns in the medical record.
  • Provides the following in clinic support:
      • Assists with clinic coordination and team communication to support clinic flow and patient care provided.
      • Performs specific clinical assessments or procedures for patients, if trained and competent to do so.
      • Transcribes provide orders to create nursing orders and discharge instructions.
      • Assist providers in all aspects of the consultation, treatment plan execution and follow up care needs of the patient!
      • Conducts review of patient care summary with patient family prior to patient discharge from clinic visit

Qualifications

  • Graduate of an approved school of professional nursing. BSN required.
  • Three years’ experience in pediatric nursing required; experience in technology dependent clinical specialty desirable.
  • Current Illinois Professional Nursing Licensure. Certification in Pediatric Nursing, Case Management or specialty preferred.
  • Excellent written and verbal communication skills, excellent people skills, demonstrated knowledge and expertise in pediatrics and any specialty areas, as well as family education; Active membership in appropriate professional organizations; Ability to speak Spanish desirable.
  • CPR required. Only certifications issued by the American Heart Association or the American Red Cross will be accepted.
