Make a difference in the lives of New York City's older adults, their families and the community. For over 60 years PSS has performed vital and important work -- through its 10 centers, caregiver services, and affiliated programs, the agency touches the lives of over 2,000 people a day. NY Connects Brooklyn is a borough-wide program funded by NYC Aging and administered in partnership with NYSOFA. The program provides free, unbiased, and comprehensive information and assistance (IA) on long-term services and supports (LTSS) for older adults, people with disabilities, caregivers, and professionals.


The Outreach & Administrative Assistant is a hybrid role supporting both public-facing outreach and education efforts and internal program operations. This team member will help ensure the programs deliverables are met by coordinating outreach events, assisting in public education sessions, and maintaining compliance through strong administrative support and documentation quality assurance. The ideal candidate has a background in social work, human services, or public health, and is passionate about aging and disability equity, system navigation, and connecting historically marginalized communities to vital services.


This position reports to the Assoicate Director of Community Services.

Each contract year, the program is responsible for:

  • 6,000+ Information & Assistance (IA) units
  • 48 Outreach units
  • 24 Public Education (PE) units
  • Compliance with rigorous documentation and quality assurance standards

RESPONSIBILITIES:

Outreach & Public Education (60%)

  • Implement community engagement strategies to increase visibility of NY Connects Brooklyn and improve access to long-term services and supports (LTSS).
  • Coordinate and conduct outreach activities across Brooklyn, including tabling, flyering, and presentations at community events hosted by senior centers, NYCHA developments, healthcare sites, libraries, and more.
  • Co-facilitate public education sessions in alignment with NYC Aging-approved topics (e.g., Medicaid, SNAP, housing resources, elder abuse prevention), ensuring compliance with contract deliverables.
  • Track and log all outreach and PE activities using NYC Agingapproved formats, including unit type, audience demographics, location, attendance, and distributed materials.
  • Maintain a master calendar of all outreach and partner engagement events and contribute to monthly outreach strategy meetings.
  • Cultivate and sustain relationships with community-based organizations, faith-based groups, and local stakeholders to expand referral pathways and cross-promotion.


Administrative & Quality Assurance (40%)

  • Assist the Program Director in reviewing and flagging staff-submitted IA case notes to ensure alignment with NYC Aging and NYSOFA documentation standards (timeliness, linking, and narrative content).
  • Track IA units in PeerPlace and reconcile internal spreadsheets to ensure data accuracy for deliverable reporting and audits.
  • Provide administrative support for deliverable tracking, including unit tallies, follow-up logs, audit documentation, and monthly summaries.
  • Organize and maintain digital and physical records of community events, referrals, sign-in sheets, flyers, and consumer education materials.
  • Assist in scheduling internal team meetings, trainings, and outreach activities.
  • Support preparation of site visit binders, quality assurance documentation, and training compliance reports as required by NYC Aging.
  • Update resource directories, contact lists, and warm handoff/referral tracking tools as part of ongoing system navigation support.


QUALIFICATIONS AND EXPERIENCE:

  • Bachelors degree in Social Work, Human Services, Public Health, or a related field required
  • Masters degree (MSW or equivalent) preferred
  • Minimum of 2 years of experience in outreach, case management, nonprofit program administration, or system navigation for aging/disability populations
  • Familiarity with NYC's LTSS ecosystem, public benefits, housing, and health care systems strongly preferred
  • Proficiency in Google Workspace, Microsoft Office; experience with PeerPlace, Salesforce, or other CRM systems a plus
  • Strong written, verbal, and interpersonal communication skills
  • Bilingual fluency (e.g., Spanish, Haitian Creole, Russian, Mandarin) is a strong asset

ADDITIONAL INFORMATION:

  • This is a full-time position (35 hours per week)
  • Work schedule includes a hybrid structure of in-field outreach, remote admin hours, and in-office team coordination
  • Participation in NYC Aging and NY Connects trainings is required
  • The role contributes directly to contract compliance and program success as measured by outreach visibility, IA unit tracking, and quality assurance metrics
