Under the guidance of the primary care operations team, the Primary Care Nurse collaborates closely with our Primary Care Providers to deliver safe, timely, effective, efficient, equitable, and patient-centered care in a structured manner. This role involves partnering with patients, healthcare providers, and other clinical team members to enhance quality outcomes for a designated patient panel. All responsibilities will align with our mission, values, and service standards.
Position Overview:
Primary Care RN or LPN - Clinics - South County (FLOAT)
Part-time: 28 hours per week
Schedule: Monday - Friday
No weekends or holidays!
**Completion of three months of on-site training is required before transitioning to a hybrid work-from-home schedule; availability for monthly clinic meetings and 1-2 on-site visits per week is also necessary.**
This newly established role for our clinics involves the nurse working closely with Providers and managing triage calls and messages for the clinic. After training, there is an opportunity to shift to a hybrid work model.
LPN Qualifications:
Education: Diploma in Practical Nursing
Licensure: Current LPN license
Experience: Preferred 2-3 years of relevant experience
RN Qualifications:
Education: Associate of Applied Science in Registered Nursing, graduation from a registered nursing program, Bachelor’s degree in nursing (BSN), or 4 years of equivalent experience in lieu of a bachelor’s degree
Licensure: Current RN license
Experience: Preferred 2-3 years of relevant experience
