Job DescriptionTricare Home Health Services Inc. is a locally owned, family-run home health agency that has proudly served the San Antonio community since 1996. We are seeking a reliable, organized, and proactive Intake & Care Coordination Specialist to join our in-office team long-term.
This is a core operations role focused on intake, follow-up, documentation, and care coordination support. The right candidate takes ownership of their work, follows through, and understands that consistency and documentation protect patients and the agency.
Schedule & Location
- In-office position only (San Antonio)
- Monday–Friday
- Approximately 30–40 hours per week
- No field visits
Primary Responsibilities
Intake & Referral Management (Primary Focus)
- Process new referrals same day
- Request and follow up on facility records until received
- Verify insurance and track authorizations
- Coordinate intake with clinicians and schedulers
- Ensure no referral or task is left unresolved
Documentation & Compliance (Non-Negotiable)
- Document all calls, follow-ups, and actions in the EMR the same day
- Upload and organize patient records accurately
- Maintain charts that are audit- and survey-ready at all times
If it isn’t documented in the EMR, it didn’t happen.
Care Coordination & Office Support
When referral volume is low, this role remains productive by:
- Following up on missing orders and signatures
- Assisting nurses with non-clinical coordination tasks
- Closing open items in patient charts
- Supporting compliance and operational needs of the office

What We’re Looking For
Required
- Experience in a medical or healthcare office setting
(home health, clinic, hospital, intake, authorizations, or similar) - Strong follow-up and documentation habits
- Comfortable making repeated phone calls
- Organized, dependable, and detail-oriented
- Basic computer skills and willingness to learn new systems
Preferred (Not Required)
- Home health intake or care coordination experience
- Medical Assistant (MA), LVN, or RN background
- Insurance verification or authorizations experience
- Spanish-speaking a plus
A license is not required. Reliability, follow-through, and willingness to learn are essential.

This role is first and foremost an intake and care coordination position. Growth opportunities are earned through consistent performance.
Compensation
- $22–$25/hour to start, based on experience
- Opportunity to earn $27–$30/hour after 90 days
with demonstrated performance and expanded responsibilities
