Responsibilities:

• Provide comprehensive care for acute and chronic conditions.
• Work with care teams to close care gaps and meet HEDIS and COC targets.
• Focus on prevention and health education, reducing avoidable ER visits and hospitalizations.
• Document patient encounters in EMR systems, with minimal administrative burden.
  

Qualifications:

Master's degree in nursing, with certification as a Family or Adult Nurse Practitioner.
Unrestricted ARNP license in Florida.
At least 2 years' experience in primary care or value-based care (preferred).
Experience with Medicaid and underserved populations is a plus.
