This position is contingent and paid on a per visit basis; orientation and additional training will be paid at a separate hourly rate. We offer competitive wages, bi-weekly pay, and direct deposit.
Responsibilities:
- Provide initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments
- Perform assessments within 24-48 hours of receiving the initial referral
- Establish a treatment plan in consultation with the physician and revise it with the approval of the physician as necessary
- Educate patients and caregivers in a way they will understand
- Record on a timely basis all evaluation data, treatments, and patient's response to therapy intervention
- Supervise Physical Therapy Assistants as per agency policy
- Participate with staff, patients, and physicians in discharge planning activities
- Adhere to Federal, State, and Accreditation requirements including Medicare and Medicaid regulations
Minimum Qualifications:
- Physical Therapist License in good standing within the State of Michigan
- Preferred one (1) or more years of PT experience, specifically in a community/home health care setting
- Must have knowledge of Medicare/CMS guidelines
- Must have experience with Outcome and Assessment Information Set (OASIS) assessments
- Experience using an EMR (Kinnser preferred)
- Must have your own reliable transportation, valid MI driver’s license, with proof of auto insurance
- BLS through AHA or Red Cross
