Job Description:

Assist residents/patients with Activities of Daily Living (ADLs) including:
-
Feeding
-
Bathing
-
Ambulating/mobility support
-
Skin care and hygiene
-
Demonstrate effective listening skills and respond to resident/patient needs with compassion.

Obtain and record initial vital signs and weights and report measurements accurately to nursing staff.
-
Report any changes in vital signs or resident condition promptly to the Supervisor/Charge Nurse.
-
Always Adhere to Universal Precautions and infection-control procedures.
-
Monitor, track, and document resident/patient intake and output per unit protocols.
-
Maintain patient confidentiality and always support resident rights.
-
Foster positive working relationships with nursing and healthcare team members.
-
Provide resident care promptly and accurately as directed by an RN/LPN and in alignment with the care plan.

Assist with patient/resident and family orientation to the unit, facility, and services.

Demonstrate empathy, patience, and professionalism when interacting with residents, families, and staff.

Remain objective and composed when dealing with challenging behaviours or situations.
-
Show flexibility and willingness to adapt to changing care needs.

Seek support when needed and contribute to problem-solving efforts.

Display dedication to providing high-quality care to long-term care and skilled rehabilitation residents.
