Nurse Practitioner Post-Discharge In-Home Care (Mid-Cities of Dallas Ft Worth Area)


Segue Health Faith-Based Medical Practice

Segue Health, a faith-based medical practice, is seeking an experienced and compassionate Nurse Practitioner to join our team serving patients in Mid-Cities of DFW area. This role focuses on providing in-home post-discharge care management and medical treatment for patients transitioning from acute care facilities back to their homes.


Key Responsibilities

  • Conduct in-home patient visits to assess clinical status and provide post-discharge medical care.
  • Review and interpret discharge summaries and progress notes.
  • Collaborate closely with home health professionals and other members of the care team to ensure seamless coordination of care.
  • Provide medical consultations, prescribe medications when appropriate, and oversee ongoing care plans to promote recovery and reduce readmissions.


Qualifications

  • Current Texas medical license and active Medicare provider number required.
  • Nurse Practitioner certification in good standing.
  • Ability to travel within a 35-mile radius from home to see patients throughout Mid-Cities of DFW area.
  • Strong communication, clinical judgment, and documentation skills.
  • A compassionate, faith-driven approach to patient care.


Compensation:

  • Base Salary: $115,000 $135,000 (commensurate with experience)
  • Quarterly Performance Bonuses available
