Under the daily supervision of Practice Management, and in consultation with the Charge Nurse and/or Chief Medical Officer, the Licensed Vocational Nurse (LVN) provides professional standard outpatient nursing services. As a patient advocate, the LVN will also provide physical and psycho-social support to patients, their friends, and families. The LVN provides support to Provider care teams and Practice Management staff and may oversee clinical care as compatible with their licensure including monitoring of duties performed by Medical Assistant (MA) staff.
Schedule: This is a full-time position, working 40 hours per week, typically Monday through Friday, with occasional Saturday hours.

Compensation:
$33.41 - $36.51 per hour, depending on experience.
This position is represented by SEIU, with compensation and benefits determined by the terms of the Collective Bargaining Agreement (CBA). Salary is based on the wage scales outlined in the agreement. Responsibilities
- Provides nursing services under the direction of a Healthcare Provider, included by not limited to medication administration, immunizations, referrals to other providers and community health resources; and health instruction to patients or following examinations.
- Administers medication and treatment as prescribed through documented Healthcare Provider orders, including TVHC Standing Orders policy. Ensures documentation reflects care provided. Is accountable for own errors in the administration of medications and treatment and will be subject to established disciplinary measures in this regard.
- Performs encounter forms and completes electronic health record entries (EPIC) for appropriate nurse-visits (e.g., health status checks, early urine pregnancy tests, medication administration).
- Provides in-person nursing care as directed by Registered Nursing and/or Healthcare Provider as indicated (e.g., urinalysis, blood pressure check) in the provision of triage nursing care.
- Performs patient chart updates, including reconciliation of medication list; including name(s) of meds, dosages, frequency, duration and start date.
- Obtain medical report from lab resulting agencies, facilitation of communication to Healthcare Provider. Completion of any required follow up in response to lab results.
- Obtain medical report from outside healthcare providers (e.g., Specialty, Pharmacy), facilitation of communication to healthcare provider.
- Obtain patient history for unscheduled patients arriving to the clinic as needed. Performs assessment data collection and documents subjective data (Chief Complaint) and objective data (e.g., vital signs/weight). Reviews basic health history information for patients. Collaborates with Registered Nursing and Healthcare Provider to meet patient care needs.
- In coordination with MA staff, assists Healthcare Provider with in-clinic examination and procedures as appropriate.
- Coordinates with Practice Management to ensure adequate medical supplies for clinic. Maintains accurate inventories for emergency carts and clinic medications.
- In the capacity of patient advocate, assesses the learning needs of patients, develops, and implements patient education as appropriate.
- Acts as a resource and contact person for Practice Management staff.
- Attends general and medical staff meetings. Participates in quality improvement projects.
- Assists the Nursing Operation Manager, Practice Management staff on matters relating to nursing practices. Participates in development of policies and protocols for patient care.
- Performs all duties and services in full compliance with TVHC’s Service Excellence Standards.
- Performs all duties necessary for successful EHR/EPM (EPIC) entries and maintenance for patients/clients within designated time set by the organization.
- Performs other related duties as may be assigned by the Nursing Operations Manager, Practice Management leadership and/or CMO.
Requirements:
- Current, valid California Licensed Vocational Nurse license.
- Valid Cardiopulmonary Resuscitation Card (CPR).
- Minimum one year experience as LVN, in an outpatient clinic preferred.
Qualifications
- Excellent communication skills at level necessary for taking patients’ medical histories, understanding provider and supervisor instructions, and for accurately documenting patients’ medical information.
- Ability to effectively communicate with patient population and staff while demonstrating a high degree of diplomacy and tact.
- Intermediate to advanced computing and phone skills.
- Bilingual in English/Spanish strongly preferred.
- Working knowledge of “Universal Precautions” and demonstrated professionalism at all times.
- Ability to multi-task and work effectively in a high-stress and fast-moving environment
- Possess a thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA.
- Culturally sensitive and demonstrated ability and effectiveness working with ethnically diverse populations.
- Demonstrated flexibility and willingness to work on an as-needed basis in order to meet operational needs of the clinic.
- Must have ability to travel to and from various service sites as directed.
