MedAdventures is seeking a dedicated and compassionate Physical Therapist (PT) to join a Skilled Nursing Facility (SNF) team in Des Moines, WA. This is a rewarding opportunity to provide essential therapy services to adult and geriatric patients, helping them regain mobility, function, and independence in a supportive and team-focused environment.

Responsibilities:
- Evaluate and treat patients recovering from illness, injury, or surgery
- Develop and implement individualized care plans based on functional goals
- Provide direct therapy services in alignment with care plans and facility protocols
- Collaborate with rehab staff, nursing, and facility teams to support optimal patient outcomes
- Maintain accurate and timely documentation in compliance with SNF and regulatory standards
Qualifications:
- Active Washington PT license (or eligibility to obtain)
- Experience in SNF or adult rehab setting preferred
- Strong clinical reasoning, communication, and collaboration skills
- Passion for delivering patient-centered, functional care
