Pride Health is hiring a Licensed Social Worker to support our client’s Healthcare Facility in Waukesha, WI 53189. This is a 3-month contract assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Location: Waukesha, WI 53189
Duration: 3+ months Contract (Possibility of extension)
Pay rate: $34-$37/hr. On W2 (Negotiable - Pay Rate is based on experience)
Schedule: Shift start times at 8:00 AM or 12:00 PM (8.5-hour shifts - including meal break)/ Weekday and weekend coverage required. (40 hrs./ wk.)
Job Responsibilities:
- Provide direct social work and case management services to patients and their families throughout the hospital stay.
- Conduct comprehensive psychosocial assessments to identify patient needs, barriers to care, and appropriate interventions.
- Deliver crisis intervention, emotional support, and brief counseling for patients and families experiencing acute or complex situations.
- Demonstrate a strong understanding of mental health diagnoses and incorporate appropriate interventions into patient care plans.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care and discharge planning.
- Identify patient resource needs and connect individuals with appropriate community programs, services, and support agencies.
- Coordinate referrals and facilitate communication between the hospital and community providers to ensure continuity of care.
- Arrange and facilitate interdisciplinary care conferences and family meetings as needed.
- Respond to patient, family, staff, and community information and referral requests in a timely manner.
- Advocate for patients by helping address psychosocial barriers and promoting access to appropriate services.
- Assist patients and families with problem-solving, coping strategies, and wellness-focused planning.
- Maintain accurate, timely, and confidential documentation in accordance with hospital policies and regulatory requirements.
Qualifications:
- Master’s degree in social work, or higher required.
- Licensed to practice in the State of Wisconsin.
- Previous hospital social work experience preferred.
- Prior Emergency Department experience highly preferred.
