Title: Licensed Practical Nurse (LPN)
Location: Santa Fe, NM
Duration: 9 Weeks
Guaranteed Hours: 36

6:00 AM - 6:30 PM 30 minutes break Position Summary

Under the supervision of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides efficient, high-quality nursing care to achieve positive patient outcomes and satisfaction. The LPN contributes to assessments, care planning, and direct patient care, while supervising unlicensed staff in accordance with the State Nurse Practice Act.

Key Responsibilities

Patient Evaluation & Care Planning

  • Collect and document objective and subjective patient data.
  • Observe and report changes in condition to the RN.
  • Contribute to individualized care plans and implement interventions.
  • Evaluate care effectiveness and assist in discharge planning.

Direct Patient Care

  • Administer medications and treatments as prescribed by physicians.
  • Accurately document all patient interactions and clinical procedures.
  • Assist physicians with rounds, exams, and procedures.
  • Provide patient and family education regarding care and discharge instructions.

Team Leadership & Supervision

  • Assign and monitor tasks for unlicensed staff per policy and Nurse Practice Act.
  • Communicate patient information effectively across shifts.
  • Ensure delegated duties are completed safely and in compliance with facility standards.

Clinical & Operational Support

  • Utilize PointClickCare (PCC) for charting and documentation.
  • Perform drug counts at shift changes.
  • Maintain clean, safe, and well-supplied patient care areas.
  • Participate in shift handoffs, interdisciplinary meetings, and care coordination.

Compliance & Professional Conduct

  • Adhere to all legal, ethical, and regulatory requirements including HIPAA and company policies.
  • Maintain confidentiality of Protected Health Information (PHI).
  • Participate in required training, quality assurance, and performance improvement initiatives.
  • Support a culture of safety, teamwork, and respect for patients and staff.

Qualifications

  • Graduate of an accredited school of practical or vocational nursing.
  • Current LPN license through the State Board of Nursing.
  • Prior experience in rehabilitative or geriatric nursing preferred.
  • Current BLS/CPR certification required.
  • Ability to read, write, and communicate effectively in English.
  • Tuberculin Skin Test (TST) verification as required by state and company policy.

Additional Information

  • May be required to stay beyond scheduled shifts to meet patient or staffing needs.
  • Expected to participate in ongoing professional development and staff orientation.
