We are working with an outstanding home health organization seeking a hands-on Nursing Supervisor to support and oversee the clinical operations of a Medicare-certified Skilled Home Health program.

This role is ideal for an RN who enjoys coordinating skilled episodes, supporting field clinicians, ensuring strong documentation, and maintaining smooth clinical operations while performing occasional patient visits.

Key Responsibilities

• Oversee the skilled episode process from Start of Care through discharge
• Support and mentor home health nurses to ensure high-quality, patient-centered care
• Monitor OASIS completion, NOA submission, and Plan of Care signature tracking
• Review clinical documentation to ensure accuracy, completeness, and Medicare compliance
• Conduct supervisory visits and occasional patient visits (SOC, recerts, or PRN as needed)
• Provide real-time clinical guidance and support to field staff
• Participate in case conferences and interdisciplinary care coordination
• Support clinicians with documentation standards, regulatory requirements, and best practices
• Help maintain survey readiness and compliance guardrails
• Collaborate with leadership to ensure smooth episode flow and efficient clinical operations

Skills & Qualifications

• Active RN license in Pennsylvania
• Home health experience strongly preferred (Medicare-certified agency experience ideal)
• Understanding of skilled episode workflow and home health documentation
• Knowledge of OASIS, NOA processes, HIPAA, and Medicare compliance
• Strong clinical assessment skills and sound clinical judgment
• Excellent communicator who enjoys supporting and guiding field clinicians
• Highly organized with the ability to manage multiple episodes and priorities
• Interest in growing into broader clinical leadership over time
