Pride Health is hiring a Home Health Registered Nurse (RN) to support our client’s home health division in Danville, Pennsylvania. This is a contract opportunity with a respected healthcare organization providing in-home patient care services.
Job Responsibilities:
- Complete Start of Care (SOC) assessments
- Perform Resumption of Care (ROC) visits
- Conduct Recertification visits
- Deliver skilled nursing care in patients’ homes
- Accurately document patient assessments and care plans
- Collaborate with interdisciplinary care teams to ensure quality patient outcomes
- Maintain compliance with agency policies and healthcare regulations
Required Qualifications:
- Active Registered Nurse (RN) license required
- Associate Degree in Nursing or higher
- Home Health experience preferred
- Strong clinical assessment and documentation skills
- Ability to work independently in the field
Preferred Skills:
- Experience with SOC, ROC, and Recertification visits
- Strong communication and time-management skills
- Compassionate and patient-focused approach to care
