Shift: 8:00am - 4:30pm

*position may qualify for employee referral bonus. $10k sign on bonus with 3+ years of experience

Job Description:

PATIENT POPULATION:
The patient population served can be all patients, including geriatric, and adult. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.

POSITION SUMMARY:
Under the direction of the Unit Director, the Regulatory and Quality RN independently provides Holistic, individualized care using the Nursing Process for patients routinely seen on the Unit, demonstrates an advanced level of Clinical competency, Nursing judgment, teaching skills, and a commitment to improving the quality of patient care and independently performs general and Unit specific Clinical skills.

Works 30-40% of workday working through the Case Management process in DP/SNF to ensure a maximum census and maintain a waiting list following established processes. The exception to this will occur on shifts where the RN is assisting with patient care for any reason. Works closely with MDS Coordinator with all admissions to ensure new resident Care Plans are in place and works closely with admitting RN to ensure full compliance with all established guidelines during and post resident admission.

Is responsible for the DP/SFN Resident Case Management process while working closely with the DP/SNF leadership team through the resident selection process. Monitors resident placement needs using established electronic processes and maintains appropriate daily documentation in resident disposition workbook.

Is responsible for the assessment, treatment or care appropriate to the ages of the patients served. Demonstrates the ability to obtain and interpret information in terms of the patient's needs. Possesses knowledge of growth and development and has an understanding of the range of treatment needed for all patient groups.

Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

Two (2) years of Clinical experience a minimum of two (2) years in a management role is required.

Bachelor's of Science in Nursing required or in progress to be achieved within 3 years of acceptance into the position.

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required.

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

To perform this job successfully, an individual should have basic computer skills.


LICENSURE/CERTIFICATIONS:
To perform this job successfully, an individual must have and maintain current licensure as a Registered Nurse in the state of California.

Must have and maintain current American Heart Association (AHA) BLS certification or obtain within orientation process, prior to hands-on patient care.

Responsibilities and Essential Functions:
*Indicates Essential Function

1 * Is responsible for the planning, implementing, monitoring, evaluating and documenting of Administrative and Management duties pertinent to the Department.
2 * Serves as a Clinical Resource person for staff, new orientees and students during the shift.
a. Maintains current knowledge of Advance Clinical Skills.
b. Demonstrates knowledge of Hospital and departmental policies and procedures.
c. Supports all Nursing Department Programs.
d. Assists Department Director, as needed.
3 * Coordinates staffing needs with the Director and/or the Staffing Coordinator/Nursing Supervisor.
a. Ensures accurate reporting of census and patient acuity for staffing purposes.
b. Arranges for replacements when staff is absent due to illness or personal reasons.
c. Works with Director to maintain Ancillary Staff needs, i.e. travelers, registry, staff, to maintain patient/nurse ratios.
d. Assists in the orientation of new staff.
4 * Acts as a Liaison for Director/Staff related to customer/physicians complaints/problems. Assists staff/physician in problem solving. Ensures resolution of problems and notification of Director. Facilitates problem solving and promotes cooperative communication.
5 * Coordinates essential staff functions and Unit resources during crisis situation.
6 * Actively participates in achieving and maintaining department compliance with all state, federal and third party regulatory agency standards. Is accountable for maintain your department at "Survey Ready" status at all times.
a. Documentation reflects compliance with established Hospital policies and procedures.
b. Performs general and Unit-specific skills independent of supervision.
c. Participates in Quality Assurance activities and integrates findings into practice.
d. Conducts, monitors, and articulates outcomes to staff and management.
e. Makes recommendations for improvement in patient care.
f. Assists in formulating quarterly report.
g. Reports Quality or Risk Management issues through appropriate lines of authority, i.e. Administrative or Clinical.
7 Identifies and conveys Unit Education needs to the appropriate resource person.
8 Adheres to OSHA Guidelines/Standard Precautions as described in Hospital policy.
9 Demonstrates participation in Patient Care Conferences.
10 Demonstrates adaptability/flexibility in meeting Unit needs.
a. Demonstrates adaptability/flexibility to assist in unusual clinical situations, i.e. temporary reassignment, changes in patient activity/reassignment, changes in patient activity/reassignment and life threatening situations.
b. Promotes an atmosphere to open communication with respect for the individual.
c. Accepts and responds to changes in a positive fashion.
d. Interpersonal relationships demonstrate an understanding of personal responsibility to represent Nursing and Sierra View Medical Center in a courteous, helpful and productive manner.

11 * Represents appropriate practice and directs others to follow or reference Hospital policies, procedures or Standards of Nursing Care.
a. Models appropriate communication and assists other staff in the identification of appropriate communication channels and methods.
b. Models and articulates the role of Professional Nursing to other members of the Health Care Team.
c. Serves as a Clinical Resource and provides direction in patient care when working with Aides, Techs, LPN's, students and peers.
d. Actively participates in Unit problem-solving and communication by involvement in staff/Unit rep meeting.
e. Maintains current knowledge of health Care trends that affect the Clinical Practice of Nursing on the Unit.

13 Identifies nursing diagnosis and interdependent nursing problems and selects applicable nursing care standards to guide patient care.
14 * According to established clinical policy, procedure and standards:
a. Safely performs therapeutic measures, provides direct patient care, administers medications and treatments prescribed by the physician and within the scope of practice.
b. Troubleshoots equipment.
c. Demonstrates skill in clinical problem solving.
15 Evaluates effectiveness of nursing care by:
a. Reviewing/revising the plan of care.
b. Work with MDS and Charge Nurses to complete plan of care for admits.
b. Makes recommendations to health care team members.
16 Involved in pertinent meetings, i.e. Care Plan Meetings, Policy & Procedures, IDT, QAPI, and Infection Control (as needed, in lieu of designee).
a. Conducts monitors and articulates outcomes to staff and management.
b. Makes recommendations for improvement in patient care.
c. Assists in formulating quarterly report.
d. Assists in the collection of data for the QU/PI reports.
e. Reports quality or risk management issues through appropriate lines of authority, i.e. administrative and clinical.
17 * Demonstrates knowledge of role by adherence to NPSGs as it relates to area of practice.
18 * Monitoring of all documentation requirements, including monthly medication and treatment audits, monthly audits for signatures, quarterly progress notes, licensed staff notes, notes of physicians' visits and monthly weight audits.
19 * Actively participates in achieving and maintaining department compliance with state, federal and third party regulatory agency standards. Is accountable for maintaining the department at “Survey Ready” status at all times.
a. Performs daily rounds to ensure the department's compliance with organizational policies, departmental rules, and survey related corrective measures.
20 * Demonstrates continuous improvement in measures specific to your department that benchmark the Organization’s performance against national standards and result in maximum reimbursement to the Organization.
Assists in the development of new or implementation of established patient teaching programs.
21 Assists in the orientation of new staff.
a. Serves as a clinical resource an provides direction inpatient care when working with aides, L.V.N.’s, students and peers.
22 * Assists the Director and Clinical Manager as needed. Supports the Charge Nurse by assisting with new admissions, discharges, any surgical procedures and bronchoscopies.
23 * Assumes or supports Charge Nurse role and makes patient care assignments based on patient acuity, staff qualifications and available resources.
a. Organizes and establishes patient care priorities based on changing patient and/or unit status in a cost-effective manner.
b. Responsible for the quality of care for assigned patient.
a. "Maintains resident wait list on a daily basis. Surveys established processes minimally every 2 hours per day, M-F. Immediately initiates potential resident selection worksheet with residents meeting established criteria for further drill down.

b. Holds interdisciplinary team meetings within 3 hours of identification of potential resident to complete the selection process.
