What would you do as a Nurse Care Manager - MVP
As a Full-Time Nurse Care Manager - MVP at Fourth Street Clinic, you'll play a crucial role in delivering exceptional patient care and clinical services. Your responsibilities will include conducting thorough patient intakes and admission assessments while developing tailored respite care plans. You'll provide vital nursing care, ranging from triage and wound care to chronic disease management and medication education. Collaboration is key; you will engage with community partners to ensure patients receive the respite care they need while promoting continuity of care through coordination with various healthcare teams and facilities. You’ll also develop and document patient-centered treatment plans, maintain health records, and support data tracking for quality improvement initiatives.
Additionally, you’ll assist with triage duties and scheduling, ensuring patients receive timely and appropriate care. Your commitment to maintaining licensure and fostering a clean, safe, and compliant work environment reflects Fourth Street Clinic's dedication to empowering patients and promoting independence.
Are you the Nurse Care Manager - MVP we're looking for?
To excel as a Full-Time Nurse Care Manager - MVP at Fourth Street Clinic, you’ll need a blend of clinical expertise and strong interpersonal skills. Graduating from an accredited program and holding an active RN licensure in Utah is essential, ensuring you’re in good standing with the Department of Professional Licensing. Effective assessment, organizational, and multitasking abilities will enable you to navigate the diverse responsibilities of patient care and coordination seamlessly. Familiarity with wound care and a solid foundation in triage principles are crucial for prioritizing patient needs effectively.
Your commitment to adhering to HIPAA regulations and Fourth Street Clinic safety standards will ensure compliance in all patient interactions. While experience in community or population health and working with unhoused or underserved populations is preferred, your passion for making a difference and ability to adapt will set you apart in this rewarding role.
Knowledge and skills required for the position are:
Required:
- Graduation from an accredited program; active RN licensure in Utah in good standing with DOPL.
- Strong assessment organizational and multitasking skills.
- ACLS/BLS certification (upon hire or within 90 days).
- Familiarity with wound care.
- Ability to prioritize using triage principles.
- Adherence to HIPAA and FSC safety standards.
Preferred:
- Experience in community or population health.
- Experience working with unhoused or underserved populations
