Position Summary:
Acclaim Home Health Care is seeking a dedicated and compassionate Occupational Therapist to provide high-quality care to patients in the comfort of their homes throughout the Worcester area. This role is responsible for delivering occupational therapy services, developing individualized care plans, and ensuring all required home health documentation is completed accurately and in compliance with regulatory standards.
The ideal candidate is organized, patient-focused, and experienced in home health, with a strong understanding of clinical documentation and care coordination.
Key Responsibilities:
- Provide in-home occupational therapy evaluations and treatments based on physician orders
- Develop and implement individualized care plans to improve patients’ functional independence
- Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety practices
- Complete all required home health documentation accurately and in a timely manner (OASIS, visit notes, care plans, etc.)
- Collaborate with interdisciplinary team members to ensure coordinated and effective patient care
- Monitor patient progress and adjust treatment plans as needed
- Ensure compliance with all state, federal, and company regulations
Qualifications:
- Active Occupational Therapist (OT) license in the state of Massachusetts (required)
- Valid driver’s license and reliable transportation (required)
- Prior home health experience required
- Strong documentation and organizational skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage a flexible schedule
