Job DescriptionAbout the Role:Join Bellestar Home Health Care Inc as a Home Health Liaison and play a vital role in connecting patients with essential home health services. We are dedicated to providing compassionate care and improving the quality of life for our clients in Roseville, CA.

Responsibilities:

• Act as the primary point of contact between patients, families, and healthcare providers.
• Conduct assessments to determine patient needs and coordinate appropriate care plans.
• Facilitate communication and collaboration between multidisciplinary teams.
• Provide education and support to patients and their families regarding home health services.
• Monitor patient progress and adjust care plans as needed.
• Maintain accurate and timely documentation of patient interactions and care plans.
• Participate in community outreach to promote home health services.
• Ensure compliance with healthcare regulations and company policies.

Requirements:

• Minimum of 2 years of experience in home health, case management, or healthcare liaison roles.
• Valid California driver’s license and reliable transportation.
• Strong communication and interpersonal skills to build relationships.
• Ability to work independently and manage multiple priorities.
• Knowledge of healthcare regulations and home health services.
• Compassionate demeanor with a passion for helping others.
• Proficient in using electronic health records (EHR) systems.
