Hospice Chaplain

Trellis Supportive Care is seeking a Hospice Spiritual Care Counselor to provide individualized hospice pastoral services in patients homes and facilities in Winston Salem and surroundings areas. Trellis Supportive Care has made it our mission to provide compassionate care for individuals living with a life-limiting illness and their families, through quality medical, emotional, spiritual, and social support wherever care is needed. Our staff has grown from a dozen individuals to over 350 specially trained professionals who work as a team to help patients live fully and comfortably, with hope — even during life’s most challenging time.

Schedule:

  • Full-time - 40 hours per week - Monday-Friday 8:00 am – 5:00 pm; with On-Call rotation
  • Start and end your day from your home - flexibility in schedule
  • Walnut Cove office - Serving Stokes County and surrounding areas

Position Requirements: Completion of a Master’s degree from an accredited theological school/seminary. Appropriate endorsement within his/her denomination (i.e., ordination). Completion of some studies in Clinical Pastoral Education. Two years of ministerial experience. Ability to work effectively within an interdisciplinary team setting and across denominational/faith lines with a minimum of supervision. Possess excellent interpersonal communication skills, both oral and written. Demonstrates excellent organizational skills with the ability to handle a variety of tasks and to assign priorities to those tasks. Ability to work a flexible schedule as needed.

Position Duties and Responsibilities: Assist with the assessment of spiritual needs/concerns of patients and families by direct communication with the individuals, by communication with the patient’s clergy, or by information obtained by other Hospice personnel. Complete a Spiritual Assessment and develop an appropriate Plan of Care to meet the needs of the patients and families that have requested a Chaplain. Help to ensure that spiritual support is available to patients and families in keeping with their wishes and desires. Provide consultation, education, and support for other members of the Hospice team, as needed. Provide assistance to the patient and family in the utilization of the appropriate community resources and programs. Assist with Chaplain on-call duties. Communicate and collaborate with the IDT regarding patient care issues. Attend IDT meetings prepared to discuss the spiritual needs of the patient and family and to collaborate with other team members. Perform other duties as assigned by supervisor.
