Job Description
Prefusion Health is seeking a highly experienced Patient Care Coordinator to serve as a central operational and communication leader within our regenerative and performance-focused medical practice. This role requires exceptional organizational skills, elite patient communication, and the ability to proactively manage complex clinical workflows in a fast-growing, data-driven health optimization environment.
The ideal candidate brings 10+ years of experience in medical facilities, a strong background in health, wellness, or regenerative medicine, and thrives in a role that blends patient advocacy, staff coordination, and operational leadership.
Key Responsibilities
Patient Experience & Care Coordination
- Serve as the primary liaison between patients, medical providers, and clinical staff
- Guide patients through treatment plans, regenerative therapies, labs, IV therapy, peptides, and hormone optimization programs
- Ensure a concierge-level patient experience from intake through follow-up
- Proactively identify patient needs, concerns, and opportunities to improve outcomes and satisfaction
Clinical & Operational Support
- Coordinate daily schedules, procedures, follow-ups, and documentation across providers
- Maintain accurate and compliant patient records in EMR and internal systems
- Ensure smooth execution of care plans, lab timelines, and therapy protocols
- Support onboarding of new patients and integration of new clinical services
Communication & Team Leadership
- Communicate clearly and professionally with patients, physicians, nurses, and support staff
- Act as a bridge between ownership and staff, ensuring alignment on priorities and execution
- Assist in training, workflow optimization, and standard operating procedures
- Uphold Prefusion Health’s brand standards, professionalism, and culture
Organization, Innovation & Growth
- Maintain elite organizational systems for scheduling, follow-ups, and patient tracking
- Identify workflow inefficiencies and proactively implement improvements
- Support growth initiatives, new service launches, and patient retention strategies
- Contribute innovative ideas to enhance patient engagement and operational efficiency
Required Qualifications
- Minimum 10 years of experience in a medical facility (required)
- Background in health & wellness, regenerative medicine, concierge medicine, or performance-based clinics strongly preferred
- Proven experience in patient coordination, care management, or clinical operations
- Exceptional interpersonal, written, and verbal communication skills
- Highly organized, detail-oriented, and process-driven
- Proactive, self-directed, and solution-oriented mindset
- Comfortable working directly with ownership and senior leadership
- Strong understanding of medical professionalism, confidentiality, and compliance
Preferred Experience
- Regenerative medicine, IV therapy, peptides, hormone optimization, or functional medicine
- High-end concierge or cash-based medical practices
- EMR systems, scheduling platforms, and patient CRM tools
- Growth-stage or multi-location medical practices
