Health Homes Case Manager Harmonia Collaborative Care
Looking for case management work that is organized, supported, and realistic?
At Harmonia Collaborative Care, our Health Homes Case Managers help individuals navigate complex systems with consistency, compassion, and follow-through. This role is ideal for someone who values structure, clear expectations, and meaningful work without burnout.
A valid NYS driver’s license and reliable transportation are required.
What You’ll Do
• Coordinate care for individuals enrolled in the Health Homes program• Develop and maintain person-centered care plans• Build strong, consistent relationships with members and service providers• Connect individuals with medical, behavioral health, and community resources• Complete timely and accurate documentation• Work independently while supported by a collaborative team and leadership

• Have a bachelor’s degree in human services or related experience• Are organized, dependable, and communicate clearly• Appreciate structure and manageable workloads• Value professionalism and person-centered care
