Job Summary
We are looking for a qualified Medical Case Manager to join our team! You will play a crucial role in evaluating our consumers medical documentation in collaboration with caregivers, family members, and medical professionals, etc.

Responsibilities
- Implement standards of care for medical social work services
- Participate in consumers plans of care.
- Perform internal audits and review of all medial related documentation
- Assess the psychosocial status of the patients as related to their illness
- Review follow-up visits to assess and continue the plans of care
- Plan interventions with team members based on consumers' needs and findings
- Maintain accurate and up-to-date records
Qualifications
- Minimum one year of experience in health care and social work
- First Aid/CPR certification
- Valid drivers license
- Experience in Human Service, Case Management and Medical industries is a plus!
- Experience working independently and managing systems
- QMAP Certified
