You will play a crucial role in evaluating our consumers medical documentation in collaboration with caregivers, family members, and medical professionals, etc. Operating with professional expertise and deep care for the at-risk adults we serve, you are a natural problem solver and self-starter. You enjoy working in fast-paced environments that afford you the autonomy to bring your best. Responsibilities 
<ul>
<li>Implement standards of care for medical social work services</li>
<li>Participate in consumers plans of care.</li>
<li>Perform internal audits and review of all medial related documentation</li>
<li>Assess the psychosocial status of the patients as related to their illness</li>
<li>Review follow-up visits to assess and continue the plans of care</li>
<li>Plan interventions with team members based on consumers' needs and findings</li>
<li>Maintain accurate and up-to-date records</li>
</ul>
Qualifications 
<ul>
<li>Minimum one year of experience in health care and social work </li>
<li>First Aid/CPR certification</li>
<li>Valid drivers license</li>
<li>Experience in Human Service, Case Management and Medical industries is a plus!</li>
<li>Experience working independently and managing systems</li>
<li>QMAP Certified</li>
