Cuready Healthcare Staffing is seeking a qualified Registered Nurse - Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
\n\nResponsibilities\n

- Coordinate and implement patient care plans and discharge planning.
- Conduct needs assessments and arrange for Durable Medical Equipment (DME) and transport.
- Collaborate with interdisciplinary teams and participate in family meetings.
- Handle patient care coordination including for HMOs and IPAs.
- Ensure compliance with HIPAA, OSHA, and CMS guidelines.
- Support patients and families regarding advance directives and community resources.

\n\nQualifications\n
- \n
- Current California RN license, or pending with approval upon start.
- Minimum of 2 years of acute medical Care Manager experience.
- Prior experience with Epic charting system required.
- Experience in Trauma Level I or Level II preferred.
- Demonstrated experience in care coordination and discharge planning.
- Ability to manage a patient ratio of up to 1:25 efficiently.
