EDUCATION AND QUALIFICATIONS:
- Currently licensed in the State of Florida as a Registered Nurse
- One (1) year of Hospice- related nursing experience preferred
ESSENTIAL JOB RESPONSIBILITIES:
- Acts in a professional and responsible manner within and outside the company; contributes a positive image; utilizes positive interpersonal skills.
- Adjusts to shifting priorities, ambiguity and rapid change; demonstrates flexibility, correlating with the needs of the patient/family/facility.
- Demonstrates leadership and utilizes critical thinking skills in assessing prospective admissions. Evaluates each case on an individual basis and maintains compliant with admission criteria as identified by the LCD’s.
- Effectively communicates with patient, family, supervisor and physicians in facilitating a smooth admission.
- Initiates the patient’s plan of care and educates patients and families on the services offered by GHS.
- Provides ongoing assessments of physical, psychological, social and spiritual needs of the patient/family unit and initiates appropriate intervention; documentation is accurate and returned to Intake Coordinators within 24 hours.
- Anticipates future needs of the patients and is pro-active in ordering appropriate DME and requesting nursing/CNA/Volunteer Services.
- May serve as a Hospital Liaison when requested
- Will complete Home Health Start of Care as assigned.
- Rotating holiday coverage as assigned, minimum of one holiday shift per year.
- Other duties as assigned
- Maintains a professional relationship and serves as a liaison with other disciplines/facilities.
- Attends to patient and families complaints/concerns and works diligently to promote timely effective resolution.
- Educates the community regarding Hospice care, philosophy and concept through a case by case consultation with individuals in the community, educational in-services or seminars when requested.
- Possess the ability to multi-task and exhibits adaptability and flexibility in maintaining compliance in correlation with GHS standards.
- Ensures all patient care and documentation of care is compliant with GHS standards, Florida State and Federal Regulations.
- Ensures professional standards of Hospice nursing practice are maintained.
- Serves as a professional resource to the community.
- Responsible to ensure all Florida State CE requirements are maintained for valid licensure.
SKILLS AND COMPETENCIES:
- Maintains up to date skill sets as required to care for patients.
- Possesses ability to multi-task; exhibits adaptability and flexibility in meeting Gulfside Hospice & Pasco Palliative Care objectives.
- Ensures compliance with all patient care, meets documentation standards and regulations.
- Assists in maintaining federal and state regulatory compliance. Adheres to GHS policy and procedures.
- Proficient in Electronic Medical Records.
