Job Description:

Seeking an experienced Home Health RN for a 13-week assignment. The nurse must be flexible to cover all territories under the Lake Oswego branch and must meet productivity requirements.

Requirements:

• Active RN license in Oregon – Required
• 2+ years of Home Health experience – Required
• Experience with Homecare Homebase (HCHB) – Required
• OASIS proficiency – Required
• Must have prior HCHB experience or training before start
• Reliable transportation, valid driver's license, and insurance – Required
• CPR certification – Required
• 1–2 years RN experience in a clinical care setting – Required
• CHPN certification – Preferred
• – Preferred

Education:

• Graduate of an accredited ADN or Nursing Diploma program – Required
• Bachelor of Science in Nursing (BSN) – Preferred
