What You’ll Experience

Your Role
- Provide comprehensive primary care to older adult patients
- Take ownership of outcomes while delivering personalized, relationship-driven care
- Serve as a health coach, guiding patients toward sustainable lifestyle changes
- Coordinate specialty and hospital care while maintaining continuity
- Lead and collaborate with your care team
- Supervise and support Advanced Practice Providers (APPs)
- Partner with leadership on quality, experience, and cost goals

- Reduced hospital admissions

What You Bring
- MD or DO (Board Eligible/Certified in Internal Medicine, Family Medicine, or Geriatrics)
- Active (or eligible) Virginia medical license & DEA
- Passion for value-based, patient-centered care
- Collaborative, team-oriented mindset
Compensation & Benefits
- $250,000 – $306,714 base salary (based on experience)
