Qualifications
- Active Physical Therapist (PT) license in the State of Florida
- Graduate of an accredited Physical Therapy program (DPT preferred)
- Strong knowledge of rehabilitation techniques and patient care planning
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively within a healthcare team
- Compassionate, patient-focused, and dependable
Preferred Qualifications
- Experience in home health, rehabilitation, geriatrics, or outpatient care
- CPR/BLS Certification
- Experience working with elderly or mobility-impaired patients
- Familiarity with electronic medical records (EMR) systems
Requirements
- Valid driver’s license and reliable transportation
- Ability to travel to patient homes within assigned service areas
- Must pass background screening
- Authorized to work in the United States
- At least 2 year work Experience as Physical Therapist
Job Responsibilities
- Assess patient mobility, strength, and physical function
- Develop individualized therapy treatment plans
- Assist patients in improving mobility, balance, and pain management
- Educate patients and families on exercises and recovery techniques
- Document patient progress and communicate with healthcare providers
- Provide compassionate in-home therapy services to support recovery and independence
